Baptist Medical Group Primary Care - Live Oak, provides comprehensive health care for individuals and families. Our practice is centered on quality, one-on-one patient care between you and board certified family medicine providers.

Medical Services
We offer a full scope of treatment options for a wide array of conditions, including minor aches, pains, bruises, scrapes, bites, colds, sinus infections, ear aches, diabetes management and other ailments.

Our Staff
While visiting our office, you will meet our team. The front office staff will welcome you and help with paperwork. They will also coordinate with clinical staff on your arrival and care needs.

Appointment Preparation
To save time, we encourage all first time patients to download our new-patient paperwork forms on this page. Print the form, fill in all information and bring the complete packet to your appointment. There may be additional items to process upon arrival.

What to Bring
Bring to the appointment your completed new-patient paperwork, a valid photo ID, current insurance card, your current prescription bottles, supplements and over-the-counter medications you’re taking. Patients are always welcome to bring a significant other, family member or caregiver with them to appointments.

Arrival
We recommend patients arrive 15 minutes prior to appointment time. This helps the front office team provide a smooth patient transition from the waiting area to consultation and caregiving.

Missed Appointment
We understand that sometimes unexpected occurrences may arise that would cause you to cancel or miss an appointment. Please let us know immediately if you run late or you are unable to keep your appointment. Please notify the office at least 24 hours in advance whenever you must cancel or reschedule your appointment.

Most Insurances Accepted
We accept most major insurance plans. Please call our office to learn the financial details specific to your insurance coverage.

Our Financial Policy
Our financial policy states that co-payments, co-insurance, deductibles and any outstanding balances are due at the time of service. Please adhere to this policy and pay those items (as defined by your insurance policy) during your appointment. We accept payments in cash, check or credit card.

You are financially responsible for the care you receive, even if your insurance does not pay. If you have questions, contact our office. Our staff will do everything we can to serve you.
