What is Jay Hospital Skilled Care?
Skilled care, also referred to as swing bed, is for a patient who needs short-term rehabilitation care beyond their acute hospital stay. Jay Hospital Skilled Care will allow the patient to transition from an acute hospital care setting to rehabilitation/skilled nursing care with the added benefit of being in a hospital.
At Jay Hospital, our goal is to help you return to your home independently or with the support of other community resources, such as home care or outpatient therapy. Our mission is helping people throughout life’s journey and in doing so, we are proud to offer Jay Hospital Skilled Care to our community.
What are the advantages of the Jay Hospital Skilled Care?
When you reach the point in recovery that acute care hospitalization is no longer necessary, but you are not physically able to go home, admission to Jay Hospital Skilled Care is a step forward. We place a heavy focus on rehabilitative therapy such as physical and occupational therapy. There are many benefits of our physician led hospital-based skilled care program.
- You’ll stay in our fully staffed hospital with inpatient and emergency care in the event of a medical crisis.
- Recover fully supported by many primary care providers who oversee the patient’s care.
- Specialty services are available such as IV therapy and wound care.
- Your treatment plan will be tailored to your personal needs with active involvement by physicians, rehabilitation therapists, nursing, dietary and social services.
How do I get started?
Once it is determined that you require skilled care, a referral will be made to Jay Hospital Skilled Care.

What will I do when my skilled care needs end?
When patient goals have been met, the Jay Hospital Skilled Care team will work with you and your family to arrange for all of your discharge needs. Many patients are able to return home.
How do I pay for skilled bed care?
- Skilled care is covered by Medicare, Medicare Advantage and many other plans.
- When you are referred to our program, or just want to make an inquiry, we will review your insurance coverage for eligibility. Call 850.860.4028 to learn more.
What About Visiting?
We encourage visitation for our patients. They need support and encouragement. Visitation hours are 7 a.m. to 9 p.m. If you have a visitation request outside our normal visiting hours, special arrangements may be made by contacting the nursing staff.
Family Involvement
We strongly encourage family members to be actively involved in their loved one’s care. We welcome the opportunity to meet with families about the care and goals while your loved one is a patient at Jay Hospital Skilled Care.
