What is FollowMyHealth?

FollowMyHealth is Baptist Health Care’s patient portal. It is a secure, online platform designed to help you access and manage your health information when it is most convenient for you.

Is there a fee to use the portal?

The FollowMyHealth patient portal is a free service provided by Baptist Health Care.

How will I know when something has been added to my portal?

Log on at any time to review your information. You also may decide to receive email and/or text notifications when information is revised or added to your account. Select "My Account," then "Preferences" and customize your notification settings to meet your needs.

Do I need any special equipment to use the portal?

FollowMyHealth can be accessed from any computer, tablet or mobile device with an Internet connection. The FollowMyHealth application also is available for iOS and Android devices.

Enrolling

How do I set up a portal account?

Sign-up is simple. You will receive an invitation in your email address upon hospital discharge or following a physician office visit. During the sign-up process, you will either link Baptist Health Care to your exisiting FollowMyHealth care account that you’ve had previously with another provider or create a new account if you’ve not had one before. You may receive additional emails during and after the registration process notifying you of successful completion. Your identity will be verified, and you will be connected to your health record at Baptist Health Care. Refer to the question “After I get my account set up, when and how do I log in?” for details on how to access your account after you’ve registered.

What if I don't receive the invitation email?

To ensure that the FollowMyHealth invitation email reaches you, please check your "spam folder" and add noreply@followmyhealth.com as an approved sender.

What is my username? My password?

You choose your own username and password when you create your portal account online.

After I set up my account, when and how do I log in?

Bookmark the website to make it convenient to visit on a regular basis. For added convenience add the FollowMyHealth app to your mobile device. A link to the portal also is available at visit www.followmyhealth.com/Login.

Log in whenever you want to review changes, make updates, send messages to your physician, request prescription refills and more. You may also decide to receive email and/or text notifications when information is revised or added to your account. Select "My Account," then "Preferences" and customize your notification settings to meet your needs.


PASSWORD / LOG-IN ISSUES

What if I forget my password?

If you don't remember your password, choose FMH Secure Login, enter your username and select "Reset." You will then receive an email with instructions about how to complete this process. If you originally signed up using another account (Facebook, Google, Yahoo or LiveID), select the appropriate icon and follow the instructions.

Please note: If you request three password resets in a 24-hour period and do not reset the password, this option will be locked for 24 hours.

What if I have trouble logging in or using the portal?

Call the FollowMyHealth help line at  888.670.9775 or send an email to noreply@followmyhealth.com. Support is available Monday through Friday from 8 a.m. to 8 p.m.

CONFIDENTIALITY AND PRIVACY

Will my information in the portal be secure?

Yes, your personal and medical information in the FollowMyHealth portal will be kept confidential. FollowMyHealth uses a secure, encrypted connection that meets the highest industry standards. All health data is stored on a secure server. Only you will be able to see or gain access to the information in your portal. We will not share your personal information with any outside party not affiliated with our organization.
